Referral and Admission
The Admission Process is very important to us. At WRH we want to make sure the transition to our facility is potentially seamless.
Admission to Weslaco Rehab Hospital is possible once the patient’s treating physician has submitted a referral and a rehabilitation clinical assessment has been completed by our Clinical Assessment Coordinator.
After the referral has reached our facility, our Clinical Assessment Coordinator will schedule a rehabilitation pre-screening and communicate with the transferring facility case manager or attending physician/nurse about the admit date. Our staff will access any medical records needed and coordinate the necessary arrangements for the transition to our facility. If the patient is admitted from their home, it is their responsibility to make transportation arrangements. During the course of the clinical evaluation, our Financial Coordinator will also initiate the verification for insurance benefits.
To be eligible for Admission, it is important that the patient meet certain criteria that will enable us to provide a rehabilitation program that will improve their quality of life. A qualifying patient must:
- Be medically stable, but require nursing care 24-hours a day, seven days a week
- Require at least two forms of therapy services (Occupational, Physical or Speech)
- Have potential to tolerate at least three hours of therapy daily
- Be at least 18-years of age
- Have potential to improve function or achieve independence
Request for Rehabilitation Clinical Assessment
A Rehabilitation Clinical Assessment can be requested by anyone whether it is a physician, a nurse, a case manager, family member or potential patient. The assessment however, will not be made without the physician order or referral.
